The intent of the survey is to first "Tungo' I Estao Fino'-ta, "Learn the Status of Our Language." CHE'LU then, with the Chamorro community guidance, we will be able to move forward in preserving and maintaining the Chamorro language.

CONFIDENTIALITY

The internet is accessed worldwide including CHE'LU's website. In order to ensure our online survey is accurate with data from the San Diego and Riverside County Chamorros only we must request that you fill out your last name and your area code and phone number. This information will be emailed directly to our organization for verification purposes only.

Whether the survey is filled out online or direct, any participant between the ages of 10-17 years will need to have their parent(s) or guardian complete a parental consent form before the survey is taken.

PARENTAL CONSENT FORM

Chamorro Hands in Education Links Unity (CHE'LU Inc.) A 501 (c)(3) Public Charitable Non-Profitable Organization, is conducting a Language Survey to measure the performance of Reading, Writing, Speaking, and the Comprehension of the Chamorro Language within our San Diego Chamorro Community.

CHE'LU Inc. recently was awarded a Native Language Preservation and Maintenance Grant from the Administration for Native Americans. The Language project entitled, "Tungo' i Estao Fino'-ta" "Learn the Status of Our Language," is a one year project to analyze the status of the Chamorro language in the San Diego metro area. Results will then be distributed to 15 key stakeholders.

CHE'LU Inc. is looking for participants' of various age groups including (10-17 years of age.) As an individual under 18 years of age, Parental Consent is necessary for any minor to participate in conducting of this survey. NO PERSONAL INFORMATION will be asked of and has no use to the conducting of this survey.

By reading and signing below Parents and/or Guardians:

UNDERSTAND that ALL and ANY answers provided is and will be CONFIDENTIAL, and that no information that could lead to the identification of any individual will be asked or used on any surveys or reports for the Language Survey Project, and will not be disclosed to the Survey Project Team or to any other party.

UNDERSTAND that only information (A. Social Data: Demographics: Age, Gender, Ethnicity) pertinent for the purpose of the Language Survey will be asked of and used to compile data strictly for the Language Survey.

UNDERSTAND that participation is VOLUNTARY, that Parents/Guardians or Minor Participant can choose not to participate in the Language Survey, and that Parents/Guardians or Minor Participant can freely leave at any time of the Language Survey.

I hereby as Parent/Guardian of Minor Participant sign and give full consent for the participation of this Language survey and UNDERSTAND that all questions asked and information provided is strictly for the Language Survey and is CONFIDENTIAL.
